The Requirements for Passing are stringent. Failure consists of any of the following:

     Any posterior displacement of the lens completely through the lens holder.
      This would indicate that the lens was able to flex;

     The lens must not fracture, that plainly would indicate that the lens was not
      strong enough to withstand impact;

     Any detachment of a portion of the lens from its inner surface, for example,
      no chips or pieces of lens may flake or come off the lens;

     Any penetration of the lens – meaning that the lens must hold back the ¼”
      steel ball. The ball may not pass through the lens.
